DENTAL BENEFIT

Promoting good oral health.

With our dental coverage, there’s no waiting period—so you can start using your benefit as soon as you need it And low or no copays and coinsurance amounts make important preventive care affordable.


HOW TO USE THIS BENEFIT:

1. Learn what your Generations Advantage plan covers 

2. Check to make sure your dentist is in the network If required by your plan.

Use the Northeast Delta Dental (NEDD) search tool, or call 1-800-832-5700 (TTY: 711).

3. Present your Generations Advantage Member ID Card at your dental visits. Your in-network dentist will submit claims for you.

WELLNESS WALLET BENEFIT

Now covers more fitness-related gear and fees!

Our unique Wellness Wallet benefit reimburses for a wide range of eligible indoor and outdoor equipment, classes, membership fees, acupuncture/naturopathy, weight and nutrition management programs, eyewear, Apple Watch®, and much more!


HOW TO USE THIS BENEFIT:

1. Check our Wellness Wallet Benefit Page to see if the item or service you want to purchase—such as a kayak or bike repair— qualifies for reimbursement.

2. Purchase the qualifying item or service, make a copy of your itemized receipt and proof of payment.

3. Download, print, and complete the Wellness Wallet Member Reimbursement Form from our website.

4. Mail to the address on the form and watch your mailbox for the paper check you’ll receive in 4–6 weeks

HEARING AID BENEFIT

Helping you stay connected and engaged.

We’ve partnered with Amplifon® to offer you discounts on a wide range of hearing aid devices and services.

HOW TO USE THIS BENEFIT:

1. Learn what your Generations Advantage plan covers 

2. Please see your individual plan details below.

3. Call Amplifon at 1-855-533-7486 (TTY: 711) to get started.

4. They will explain the process and help you find a hearing aid provider near you.

5. They will send information to you and the provider to make sure your discount is applied.

VISION EXTRA BENEFIT

Preserving your best vision is an important part of taking care of your overall health.

Your Martin’s Point Generations Advantage plan offers extra vision-related benefits that go beyond Original Medicare:

  • $0-Copay Annual Routine Vision Exam: You pay a $0 copay for one routine vision exam per year. This benefit includes a diabetic (dilated-pupil) eye exam for people with diabetes and glaucoma screening for those at high-risk for glaucoma. See your Evidence of Coverage for details.
  • Eyewear Reimbursement: Your plan reimburses up to an annual amount for prescription lenses, frames, and contact lenses. Amount varies by plan.

HOW TO USE THIS BENEFIT:

1. Check your benefit maximum to see your plan’s annual reimbursement amount.

2. Purchase your qualifying items (prescription lenses, frames, and/or contact lenses) and make a copy of your itemized receipt and proof of payment.

3. Download, print, and complete the Eyewear Reimbursement Form from our website.

4. Mail to the address on the form and watch your mailbox for the paper check you’ll receive in 4-6 weeks.

OVER-THE-COUNTER BENEFIT

Save on everyday health needs.

You may purchase from a selection of over 350 eligible, CVS-brand items up to your plan’s allowed amount, which refreshes quarterly. You can make multiple transactions per quarter up to your quarterly maximum. Unused amounts do not roll over to the next quarter. Some items may have quantity limits. 1. Browse our catalog of eligible items available or purchase for home delivery by visiting our CVS Over-the-Counter (OTC) Page.

Free, unlimited chats with a nutrition coach from the comfort of your home!

We’ve teamed up with FoodSmart to offer you a program that is tailored to your needs and preferences—from nutrition tips for managing a medical condition, to simply wanting to eat healthier, your Foodsmart nutrition coach and other resources can make healthy eating easier.


HOW TO USE THIS BENEFIT:

2. OR by calling FoodSmart at 1-888-837-5325 to schedule an appointment with a nutrition coach.
