Dental Plus for Essential (HMO-POS) Plan

Optional coverage for restorative care needs.

Martin's Point Generations Advantage Dental Plus is optional supplemental coverage for basic and major restorative services for a monthly premium of $69. It is administered by Northeast Delta Dental (NEDD). Once you've enrolled and your Dental Plus plan becomes effective, there's no waiting period and no deductible—so you can start using your benefit as soon as you need it and no office visit copays and low coinsurance amounts make important dental care affordable.

Dental Plus benefit amounts

Expenses paid by the plan for covered dental services count toward your Calendar-Year Maximum benefit.

Dental Plus

Calendar-Year Maximum: $1,500
Category A | Diagnostic/Preventive: This category of coverage is already included with your Generations Advantage Essential (HMO-POS) plan. It is not included in the below Dental Plus benefits.
Category B | Basic Restorative: 50% coinsurance
Category C | Major Restorative: 50% coinsurance

Diagnostic

NOTE: Category A Diagnostic/Preventive services are covered in the dental benefit included in your Generations Advantage Essential (HMO-POS) plan. This coverage IS NOT part of the Dental Plus optional supplemental coverage.

Preventive

One oral exam and routine cleaning per calendar year.*

*Office visit copay applies.

Restorative

Amalgam (silver) fillings*
Resin restorations on anterior teeth and the buccal surface of bicuspids only

Oral Surgery

Surgical and routine extractions

Endodontics

Root canal therapy

Periodontics

Periodontal maintenance (cleaning)**
Treatment of gum disease
Clinical crown lengthening once per tooth per lifetime

Denture Repair

Repair of a removable denture to its original condition

Emergency Relief of Pain

Brush biopsy once in a calendar year

* Additional costs may apply for resin fillings, please reach out to Northeast Delta Dental for more information.

**You may substitute a routine cleaning (Category A). Total number of cleanings per calendar year under Essential plan built-in dental coverage with added Dental Plus coverage cannot exceed two.

Check to see if your dentist is in-network.

When using the Network Provider Search Tool (administered by Northeast Delta Dental):

Choose the Martin's Point Generations Advantage Network.

Attend your dental visit.

Show your Generations Advantage Member ID card at your next appointment. Your in-network dentist will submit your claim for any cost shares.
