If your Generations Advantage plan was recently termed, you can re-enroll with us using our standard online enrollment form. It only takes a few steps:

  1. Click below to start your enrollment.
  2. Have your Medicare card ready with your number and effective dates.
  3. Select Special Enrollment Period: Plan Termination.
  4. Follow the prompts to enter your information.
  5. Choose the new plan you’d like to enroll in.
  6. Submit your application—once complete, you’ll receive a confirmation.
