2026 Generations Advantage Prime (HMO-POS) Plan
Low premiums and valuable added benefits. The “Point of Service” (POS) on this HMO adds out-of-network flexibility for most services. Includes Part D Prescription Drug coverage.
2026 Generations Advantage Select (LPPO) Plan
No PCP requirement, out-of-network coverage with lower costs for in-network services (exception: same low costs for primary and specialty care office visits). Includes Part D Prescription Drug coverage.
2026 Generations Advantage Alliance (HMO) Plan
In-network coverage with rich added benefits. Part D drugs not included. Ideal if you get your prescriptions through the VA or TRICARE®.
2026 Generations Advantage Essential (HMO-POS) Plan
$0-premium plan with comprehensive medical, hospital, and Part D Prescription Drug coverage, out-of-network flexibility, and optional supplemental dental.
