Welcome! Here’s what you need to do before your first appointment.

Follow these steps before your first appointment to ensure the best experience for your first visit with your new Martin’s Point provider.

Important Note:
If your primary care provider (PCP) is a Martin's Point provider at one of our health care center locations, please use the MyMartinsPoint patient portal. For other providers, use the patient portal provided by your primary care provider (PCP).

Before your first appointment, log in to the patient portal to complete the following forms, or you can print them out and bring them with you:

Note: If you’re completing the forms online, your health history questionnaire will be available two days before your appointment.

Please print, fill out, and send this form to your previous health care providers so your medical records will be sent to us.

Authorization to Release Protected Health Information to Martin’s Point 
 
Avoid delays in billing and authorizations for referrals or prescriptions by notifying your health insurance company of your primary care provider (PCP) change.

  • Completed General Consent, Financial Agreement, and Health History forms, if you did not submit online
  • Insurance card and copayment
  • Your medication bottles and a list of any over-the-counter medications you take
  • A list of your health questions and concerns
