In addition to the vision-related benefits covered by Original Medicare, your Martin’s Point Generations Advantage plan offers extra benefits that go beyond Original Medicare.

Understanding Your Coverage

Covered items and services

Your vision and eyewear benefits covered under Medicare Part B:

  • Medicare-Covered Eye Exam: Copay varies by plan for outpatient physician services for the diagnosis and treatment of diseases and injuries of the eye, including treatment for age-related macular degeneration.
  • Medicare-Covered Post-Cataract Surgery Eyewear: You pay 20% of the Medicare-allowable rate for one pair of standard eyeglasses or contact lenses after each cataract surgery that includes insertion of an intraocular lens. Learn More

 

Your extra vision and eyewear benefits (beyond Part B coverage):

  • $0-Copay Annual Routine Vision Exam: You pay a $0 copay for one routine vision exam per year. This benefit includes a diabetic (dilated-pupil) eye exam for people with diabetes and glaucoma screening for those at high-risk for glaucoma. See your Evidence of Coverage for details.
  • Eyewear Allowance: You receive an annual amount for prescription lenses. frames, and/or contact lenses for the purpose of correcting/improving your vision. Amount varies by plan (see below).
  • EyeMed Eyewear Discount: Available as an extra benefit through your Northeast Delta Dental plan. Learn More

Using the EyeMed discount program

Your Martin’s Point Generations Advantage dental benefit, administered through Northeast Delta Dental, not only takes care of your dental health but also your vision. Through your dental benefit, you can get discounts on frames, lenses, and contact lenses offered through EyeMed (note: eye exam is not available through this benefit)

  • Access Your Savings: To use your EyeMed discount, visit EyeMed Vision Care or call 866-246-9041. Simply say "Benefits" when prompted and specify whether you are looking for glasses or contacts. Follow the prompts to state the type of lenses you need.
  • Nationwide Retailer Network: Choose from an extensive network of retailers for your eyewear. Provide your EyeMed Group Number 9231093 at the time of purchase.

Note: The reimbursement benefit does not apply to purchases made through the EyeMed Discount Program.

Frequently Asked Questions

Can I use the eyewear allowance for an item that isn't included in the list of eligible items and services under the Vision/Eyewear benefit?

No, at this time the allowance can only be used for items specified in the Vision/Eyewear list of eligible items.


What are the acceptable forms of proof of payment?

The proof of payment must indicate what was bought, how much it cost, and how it was paid for. This can be in the form of an itemized register receipt, stamped bill, or paid statement. Cancelled checks, credit card bills, and bank statements can also be used, but they MUST show detail relating to the item purchased. We do not accept invoices, order / pick up confirmations, or shipping statements with no proof of payment. It is okay for you to use a gift card or gift certificate as the payment method, as long as the receipt indicates the payment method. 

Requests for 2024 reimbursement must be received by the plan no later than April 30th of 2025.


If I enroll in my Generations Advantage plan mid-year, do I still receive the entire eyewear allowance amount?

Yes! You are eligible to receive up to the entire eyewear allowance amount that is offered under your Generations Advantage plan even if you join the plan mid-year. As a reminder, dates for received services or purchased items must be within the current plan year in order to be reimbursed under that year's benefit.

 
What happens if I place an order, but my payment isn't processed until a later date?

If you place an order for an item, but payment is not taken until a later date, the date you placed the order is considered the date of purchase as long as your receipt shows the date you placed the order. For example, if you place an order for an item on 12/31 but payment is not taken from your account until 1/1, your purchase date would be considered 12/31 provided that your receipt lists the 12/31 order date.


Can a member use credit card points or a gift card to purchase an item?

A member can use credit card points or a gift card to purchase an item as long as the receipt shows the cash value of the item.


How do I find out if my eyewear reimbursement request has been denied?

Members receive their notification of eyewear reimbursement determination on their Explanation of Benefits documents within 30 days after the reimbursement claim is processed. Denial reasons will be listed on the Explanation of Benefits. 


How do I avoid reimbursement denials or delays?

Here are some tips to ensure the smoothest reimbursement experience:

  • Make sure your purchase is on the list of items/services eligible for reimbursement.

For mailed-in requests:

  • Use the PDF form ONLY and complete all sections—2025 purchases should be submitted using the 2025 form; purchases made in 2024 must be submitted using the 2024 form.
  • Mail form and copy of receipt/proof of payment to: Martin’s Point Generations Advantage Claims Department, P.O. Box 3003, Fargo, ND 58108

For mailed-in AND online requests:

  • Itemized receipts and proof of payment documents (COPIES ONLY) must be enclosed with mailed requests or uploaded with online requests. They must show WHAT was purchased, HOW MUCH it cost, HOW YOU PAID for it, and the DATE it was purchased.
Do I need to activate my card before using it?

No. Your card arrives active and ready to use. (Please sign the back of your card.)


Can I add my Martin’s Point Prepaid Mastercard to my digital wallet (e.g., Venmo, PayPal, ApplePay, etc.)?

The Martin’s Point Prepaid Mastercard is not compatible with digital wallet platforms. While it may appear that you have successfully added the card to a digital wallet, it will not work when you try to use it for transactions. You will need to use the card directly or use a different form of payment to complete a transaction, save the receipt, and file a claim for reimbursement.


How do I know how much of my benefits’ allowances are on the card?

Your card arrives preloaded with your full annual amount, with separate allowances for your Wellness Wallet and Eyewear benefits. As you use your card, remaining balances can be found on your Generations Advantage member portal at MartinsPoint.org or by calling Generations Advantage Member Services at 1-866-544-7504 (TTY: 711) 8am-8pm, every day from October 1–March 31 and weekdays from April 1–September 30.


Can I use the card to purchase any items on the Wellness Wallet and Eyewear lists of benefit-eligible items/services?

At this time, card payments are available for a limited number of all eligible WellnessWallet items/services and Eyewear at specific retailers. We are working to expand options for card use in the future.


Where can I use my card?

At this time, card payments for Wellness Wallet items and Eyewear purchase are only accepted at a limited list of locations—View the List. You may try the card for eligible purchases at your retailer of choice, but please be prepared with an alternate payment method in case the transaction is declined.


Can I combine my Wellness Wallet allowance amount with my Eyewear allowance amount and use the total toward an eligible purchase?

No. Your card is loaded with two separate balances—one for your Wellness Wallet allowance and one for your Eyewear allowance. These separate balances cannot be combined.

Please Note: The benefits refresh yearly. Unused balances do not carry over to the next year. The available balance is updated automatically after each purchase is completed
