At Martin’s Point Generations Advantage, we believe in the power of prevention to help you stay as healthy as possible. We encourage you to schedule preventive care visits with your primary care provider (PCP) every year. We also offer a convenient program to provide important health assessments in the privacy of your home.

How do in-home health visits work?
Martin’s Point partners with Matrix Medical Network to provide this service to you. On a date and time convenient for you, a nurse practitioner will come to your home and conduct a free health assessment. Most appointments take about 45 minutes.

At your visit, the nurse practitioner will:
» Take your blood pressure, and pulse.
» Review your medical history, current health conditions, and medications.
» Discuss your health concerns and help you make a list of discussion topics for your next doctor visit.
» Discuss ways to make your home safer or more adapted to your physical needs.

What you should know about this service.
» This is a free service.
» You are not required to have an in-home visit. The choice is yours.
» Our goal is to make sure you and your doctor have a complete record of all your medical conditions. Even members who see their doctor regularly can benefit from this visit.
» This visit does not replace the annual wellness visit or annual physical from your regular PCP.
» This information is confidential and will only be shared with you, your doctor, and Martin’s Point Generations Advantage.
» Information gathered during an in-home health visit will not change or affect you
