Thank You for Your Partnership
As we reflect on the first full year of our Community Health Worker (CHW) program, we want to express our sincere appreciation for the continued partnership with provider offices in working together to support the health and well-being of our shared members/patients.
Our CHW team actively reaches out to our Generations Advantage and US Family Health Plan populations to provide education around preventive care and to help identify community resources to address barriers to care. The focus of the team is to improve timely access to essential health services and empower members to take proactive steps in managing their health.
Preventive Services Addressed by CHWs
Some key preventive services that our CHWs have been addressing with members include:
- Cancer screenings
- Immunizations
- Bone density assessments for women who have experienced fractures
- A1c control
- Blood pressure management
- Kidney testing for patients with diabetes
Partnering to Close Care Gaps
When our CHWs reach out to your office, it is to support members in closing care gaps or to confirm that recommended care has already been provided.
