Your member ID card helps providers confirm your coverage and bill your plan for care.

Health care providers use the information on your US Family Health Plan member ID card to confirm they are part of your health plan’s network and to bill your plan for your care. Always present your member ID card when you go to the doctor or hospital, get lab work done, or have a prescription filled to ensure that you get all the benefits that your health plan provides.

Requesting an ID card

Ordering a card through your member portal is quick and avoids any Member Services call wait time. You can also download and print a paper copy for temporary use.
