How to Enroll in the US Family Health Plan

You will need the following information for each person enrolling:

Personal Information  This includes the sponsor's social security number, dates of birth for all enrollees, and your address (residential and mailing). If you are enrolling a newborn, see our Enrolling Your Newborn page for full details.

Primary Care Provider We will need to know the primary care provider information for each enrollee. To see if your provider is in network, or to find a new provider in your area, use our online Provider Search Tool.

Payment Information If you are retired military, you will need to provide your preferred payment information for the enrollment fee. Download the Electronic Payment Authorization Form.
