How to enroll in the US Family Health Plan
You will need the following information for each person enrolling:
This includes the sponsor's social security number, dates of birth for all enrollees, and your address (residential and mailing). If you are enrolling a newborn, see our Enrolling Your Newborn page for full details. |
We will need to know the primary care provider information for each enrollee. To see if your provider is in network, or to find a new provider in your area, use our online Provider Search Tool. |
If you are retired military, you will need to provide your preferred payment information for the enrollment fee. Download the Electronic Payment Authorization Form. |
