The US Family Health Plan is available to eligible military beneficiaries in Maine, New Hampshire, Vermont, upstate and western New York, and northern Pennsylvania.
Please note: TRICARE Young Adult has a different enrollment process and application than USFHP. Information about enrolling in TRICARE Young Adult Prime can be found further below.
You will need the following information for each person enrolling:
PERSONAL INFORMATIONThis includes the sponsor's social security number, dates of birth for all enrollees, and your address (residential and mailing). If you are enrolling a newborn, see our Enrolling Your Newborn page for full details. |
PRIMARY CARE PROVIDERWe will need to know the primary care provider information for each enrollee. To see if your provider is in network, or to find a new provider in your area, use our online Provider Search Tool. |
PAYMENT INFORMATIONIf you are retired military, you will need to provide your payment information for the enrollment fee. Download the Electronic Payment Authorization Form. |
Eligible dependents up to age 26 may purchase coverage through the US Family Health Plan.
When applying, you must verify you are not married and not eligible to enroll in an employer-sponsored health plan.
