Overview

Accurate Diagnosis, Documentation & Coding (ADDC) ensures our patients’ conditions are documented and coded at the highest level of specificity at least once annually and is critical to a successful, value-based care partnership. Adopting best ADDC practices includes consistently reporting active, chronic diagnoses in the “Assessment / Impression” and “Plan” to the highest level of known specificity along with supporting criteria for each condition to allow for accurate diagnosis code assignment during the encounter.
