Preventing complications through monitoring

Diabetes remains one of the most prevalent and costly chronic conditions. Complications from poorly managed diabetes—like kidney failure, blindness, and cardiovascular disease—can often be prevented with early detection and routine monitoring.

We seek your continued support to ensure our members with diabetes receive the care they need, including:

  • A1c Blood Sugar Control: Regular monitoring and management of A1c levels to reduce the risk of long-term complications.
  • Kidney Function Testing: Annual screening for nephropathy, including urine albumin creatinine testing and eGFR assessments, to detect early signs of kidney damage.
  • Retinal Eye Exams: Routine eye exams to monitor for diabetic retinopathy and prevent vision loss.
  • Blood Pressure Management: Keeping blood pressure within target ranges to reduce cardiovascular risk and kidney complications.

Tools and resources for providers

Diabetes Tracker: Available on our Diabetes Wellness Guide webpage, this tool helps educate members on the value of their diabetes screenings and helps them monitor their A1c and stay informed about upcoming screenings and appointments.

Personal Diabetes Action Plan: Downloadable guide outlining steps to manage diabetes as well as handling emergencies related to diabetes.
