The CMS Health Outcomes Survey (HOS) captures patient-reported outcomes for Medicare Advantage members with HOS measures currently making up 13% of the overall Medicare Part C Star ratings. Key measures reflect everyday clinical care and proactive provider engagement in these areas: 

  • Physical activity.
  • Fall risk.
  • Urinary incontinence.
  • Impact of mental health on functional status.

Falls.

This leading cause of injury in adults age 65+, drives hospitalizations, loss of independence, and increased mortality. Routine assessment of balance, gait, and overall activity level along with counseling on physical activity and balance/strength training, should occur across visits.

Urinary incontinence.

Affecting millions, this is associated with increased fall risk, skin breakdown, infections, and reduced quality of life and frequently goes unreported unless providers ask directly. HOS evaluates whether providers discuss treatment options, which may include pelvic floor exercises, bladder training, lifestyle modifications, medications, or referral when appropriate.

Mental health.

Closely intertwined with physical outcomes, depression/anxiety can reduce physical activity, and increase fall risk and functional and cognitive declines.  Incorporating routine mental health screening and support into patient encounters helps address the full clinical picture and promote functional improvement. 

Regular, proactive conversations make a difference.

Discussing physical activity, fall risk, urinary incontinence, and mental health in visits beyond the Annual Wellness Visit—such as chronic care management, follow-up appointments, and problem-focused visits— strengthens continuity of care and improves patient quality of life.
