Get started with the credentialing process at Martin's Point.

Forms are intended for participating providers or those currently in the contracting process.

Update Your Provider Information

If you need to update information related to your credentials, access our Provider DataPoint portal here.

Provider Credentialing Instructions

Complete the Council for Affordable Quality Healthcare (CAQH) application. Make sure all information and attached documents have been updated and (re)attested. Authorize Martin's Point Health Care to view your CAQH information.

Once the CAQH information is up-to-date, please use the Update Your Info function, to add the provider to a practice/group. We will determine if credentialing is needed and ask for the appropriate information. If credentialing is required, please allow up to 90 days for processing.

Martin's Point credentials the following provider types:

MD, DO, DDS, DPM, OD, DC, AuD and Certified Nurse Midwives (CNM)

Nurse Practitioners (FNP, ANP, PNP) and Physician Assistants (PA) who practice as a PCP

Nurse Practitioners (FNP, ANP, CNS) and Physicians Assistants (PA) that practice as a Specialist in Acupuncture only

Martin's Point does not credential:

PT, OT, ST, SLP practitioners (Martin’s Point credentials the group)

Providers who practice exclusively within the inpatient setting (e.g., hospitalist, laboratory personnel)

Nurse Practitioners and Physician Assistants who practice as specialists (with the exception of providers who are performing Acupuncture)

Covering Physicians (e.g., locum tenens, per diems)

Martin’s Point does not contract with Chiropractors (DC) for the US Family Health Plan.

Martin’s Point does not credential behavioral health providers.

All inquiries regarding behavioral health providers should be directed to the Behavioral HealthCare Program at 1-800-538-9698.

Once you are contracted and credentialed with Martin's Point Health Care, you will appear in our provider directory under your contracted line of business. Please check these directories to ensure you are setup accurately and to notify us using Update Your Info if there is an issue with your setup.
