As a caretaker, getting up to speed on the Medicare options available for your loved one can be time-consuming.  Here are some key things you can do to ensure a smooth decision-making process.

Health Insurance Portability and Accountability Act (HIPAA) regulations require that the person you're caring for must formally permit you to access their health insurance information. Each health insurer will be able to supply you with their required consent forms.

The first step is to complete these forms and send them back to the health insurance provider. Below you'll find the forms required by Martin's Point Generations Advantage.

Authorization to Release Information

Use this form to designate a representative to discuss certain aspects of your health care.

Medicare Appointment of Representative

Use this form to appoint a representative who can assist with all Medicare-related inquiries.

Your loved one's health care needs may evolve over time. It's important to stay up to date with their health benefits. With their consent, you should:

  • Review their health insurance coverage to see if it meets their current needs.
  • Assess costs such as deductibles, copays, and premiums to make sure the plan fits their budget.

Create a file of all your loved one’s medical information to make sure you’ll have access to it when you need it most. The file should contain:

  • Diagnosed medical conditions
  • A list of all doctors
  • Medical, dental, and vision insurance information
  • Medical records and history
  • Medicines and medication allergies
