I received a notice that my Generations Advantage plan is ending on December 31, 2025, and want to re-enroll in a new Generations Advantage plan for 2026.
Re-enroll Online
If your Generations Advantage plan was recently termed, you can re-enroll with us using our standard online enrollment form. It only takes a few steps:
- Click below to start your enrollment.
- Have your Medicare card ready with your number and effective dates.
- Select Special Enrollment Period: Plan Termination.
- Follow the prompts to enter your information.
- Choose the new plan you’d like to enroll in.
- Submit your application—once complete, you’ll receive a confirmation.
