Member notice

Prescription refill requests through the website form will end on 11/5/2025. Beginning November 10, US Family Health Plan members will have expanded options to manage their prescription refills. Theses options include a new simple-to-use mobile app which will replace the web-based prescription refill form, opt-in text communications, and an auto-refill service.
